Academic -- Definition of Academic Institutions Eligible for Academic Pricing on
Louts Products
Lotus Development Corporation has established academic pricing and programs to make our products available at the lowest possible cost to qualifying academic institutions, and individual students, faculty and staff at these institutions. The following organizations may be eligible for Academic Pricing on Lotus Products:
· Four year colleges and universities
· Vocational and technical schools
· Schools, pre-kindergarten through 12th grade
· Supervisory bodies like the State Boards of Education
· Education consortiums, shared computing facilities, networks and research institutes provided that the purposes of the organization are in basic support of and integral to the educational purposes of the participating, qualifying institutions.
· Special education schools and consortiums that are accredited by one of the following: accreditation council for services for the mentally retarded and other developmentally disabled persons, Commission on Accreditation of Rehabilitation Facilities, the National Accreditation Council for Agencies Serving the Blind and Visually Handicapped, Health Care Financing Administration, or Council on Accreditation of Services for Families and Children.
The organizations listed above may be public or private, but must meet the following criteria to be eligible for academic pricing:
· The School must be accredited by one of the six regional accrediting commissions of higher education listed below, one of the national associations of higher education, or one of the national professional associations listed in the National Center for Educational Statistics Education Directory, Colleges and Universities.

Hospitals
The criteria for a teaching hospital to quality for academic pricing is outlined in the Teaching Hospital Eligibility Form below. A teaching hospital may request permission to buy Lotus products at academic prices by having the educational institution complete and sign this form and submit it to their Lotus representative.
Teaching Hospital Eligibility Form
Lotus Development Corporation ("Lotus") has established academic pricing and programs in order to make our products available at a low cost to qualifying academic institutions. A qualifying academic institution is an accredited, not-for-profit school which grants degrees. The students, faculty and staff of those institutions may also purchase one copy per product per year.
A teaching hospital may request permission to buy Lotus products at academic prices by having the educational institution complete this form. In an effort to apply our policy fairly and within the confines of applicable law we request that you to complete the form below so that we may determine the exact relationship of the hospital to your academic institution.
In order to qualify for Lotus academic pricing, the educational institution must be able to answer "yes" to all of the following statements relative to the teaching hospital and then have this form signed by an officer (e.g., President, Dean, Provost) of the educational institution.
